ROLE AND PURPOSE
Grade 7:
To provide and support the delivery of high quality social work activities within Adult Social Care as part of a multi-disciplinary team. Works within the Vulnerable Adults operating model which includes the following teams:
Hospital See & Solve, Hospital Locality, Community See &Solve, Active Recovery Assessment, City Sensory &Telecare Support Service, East and West Locality.
The aim will be to focus on people’s strengths, agreeing goals and helping people to retain control of their life and have the confidence to maintain their independence. They will develop confidence to deliver person centred assessments and care and support plans that address people’s hopes and aspirations, as well as their physical / environmental needs, and support people to remain in control, resilient and independent. This is a developmental role aimed at Newly Qualified Social Workers during the Assessed & Supported Year in Employment (ASYE) where additional support will be provided.
Grade 8, all of the above plus:
To provide and support high quality social work practice with increasing decision-making autonomy in situations of increasing complexity, risk, uncertainty and challenge.
Grade 9, all of the above plus:
To provide and support expert and effective social work practice in complex situations, assessing and managing high levels of risk and providing expert opinion in relation to decisions surrounding complex cases. To take a professional leadership role taking responsibility for the learning and development of others, through leadership and management, supervision, mentoring, assessing and teaching.
There is an expectation that the social work role works within the Professional Capabilities Framework and Adult Knowledge and Skills Statements. In addition the worker at all grades practices within the Social Work England (SWE) standards.

Principal Accountabilities:
Grade 7
1. To promote and safeguard the welfare of adults at risk, families and their carers, within the ‘making safeguarding personal’ principles.
2. Undertake proportionate assessments, determine eligibility, and assess risk. Plan care and support and make the best use of available resources to enable people to live the life they choose. To ensure people are given correct financial advice and information about charging processes of the council.
3. Works in partnership with Occupational Therapy and Physiotherapy utilising their expertise in the assessment and review process. To assess, identify and source low level OT equipment where this could support an individual to live independently or as part of a wider package of care. To undertake role as an Equipment Trusted Assessor.
4. Engages in critical reflection and constructive challenge within the course of working with colleagues, other agencies and with vulnerable adults and families.
5. Actively engages in team meetings/practice forums and other relevant meetings ensuring focused and purposeful social work is undertaken with all cases.
6. Works with colleagues in the team to ensure the form of communications are clear and aligned with people’s needs. Demonstrate effective use of evidence-based social work practice. To ensure effective communication with people who have specific communication needs including learning disability, dementia, people who lack capacity and people with sensory impairment. Making sure advocacy arrangements are suitably in place.
7. Works with colleagues in the team to ensure that case recording is up to date and that required information is available for a variety of purposes e.g. , multi-agency meetings, strategy meetings, panels, court, case conferences etc.
8. Engages in the professional decision-making process with colleagues in relation to complex/high risk cases.
9. Actively engages in the professional development framework for Hull Adult Social Care (including attendance at practice development groups) and continuous learning as per relevant level of the Professional Capabilities Framework for the role.
10. Works within the legislative frameworks for Adult Social care, i.e The Care Act, Mental Capacity Act, Mental Health Act, Human Rights Act.
11. The Health and Safety at Work etc. Act 1974 and associated legislation places responsibilities for health and safety on Hull City Council, as your employer and you as an employee of the council. In addition to the Councils overall duties, the post holder has personal responsibility for their own health & safety and that of other employees; additional and more specific responsibilities are identified in the Council’s Corporate H&S policy.
12. Prepares for and attends relevant decision making forums.
Grade 8, all of the above plus:
13. Engages in and advises on the professional decision-making process with adults at risk, families and their carers, and colleagues in relation to complex/high risk cases.
14. To undertake Section 42 Enquiries into safeguarding concerns under the guidance of your line manager and the Safeguarding Team. Demonstrate an outcome focussed, person centred approach to safeguarding practice. To take responsibility for advising / supervising in complex cases and oversee suitable escalation to / progression to appropriate senior social workers and or management as suitable.
15. Prepares for and attends court as required in relation to specific cases where the engagement of legal processes is necessary.
16. Provides co-working support and advice to other staff members within the team.
17. Provides day to day supervision and development of staff.
Grade 9, all of the above plus:
18. Engages in and advises on the professional decision-making process with adults at risk, families and their carers, and colleagues in relation to complex/high risk cases. Occasionally deputises for the Practice Lead Manager.
19. Provides day-to-day supervision of staff and contributes to practice education of any social work student attached to the team. Supports the leadership team in the development of the team.
20. Actively supports and engages in practice development groups and the provision of information/professional opinion as required e.g. for policy amendments, newly developed ways of working.
21. Takes a leadership role in managing the daily activity of the team , i.e allocation, duty work.
