Our client based in Cardiff is seeking an experienced Social Worker to join their Mental Health Team, working within a community setting to support adults with complex mental health needs.
In this role, you will work with individuals, families and carers to promote independence, recovery and wellbeing, ensuring that people are supported to make informed decisions about their care. You will carry out comprehensive assessments, care planning and reviews, while managing risk and safeguarding adults in line with statutory duties.
Working within a multi-disciplinary environment, you will collaborate closely with health colleagues, partner agencies and community services to deliver joined-up, person-centred support. The role involves working with complex and high-risk cases, requiring sound professional judgement and strong analytical skills.
Key Responsibilities
Core duties include:
- Managing a caseload of adults with mental health needs, including complex and high-risk situations.
- Undertaking comprehensive assessments, identifying needs, risks and appropriate interventions.
- Developing, implementing and reviewing care and support plans to promote recovery and independence.
- Working in accordance with relevant legislation and statutory guidance to safeguard adults at risk.
- Supporting individuals to understand their circumstances and make informed choices about their care.
- Working closely with families, carers and support networks, ensuring their needs are recognised and addressed.
- Collaborating with multi-disciplinary teams, including health professionals and external partners, to provide co-ordinated support.
- Undertaking and managing risk assessments, ensuring decisions are proportionate and defensible.
- Managing and prioritising workload effectively, balancing competing demands using professional judgement.
- Preparing for and contributing to case conferences, reviews and decision-making forums.
- Maintaining accurate, up-to-date and high-quality records in line with organisational policies.
- Contributing to team development, service improvement and practice standards.
- Providing guidance or informal support to less experienced staff where appropriate.
- Working within budget frameworks to help identify appropriate and cost-effective support arrangements.
Qualifications & Experience
Professional Qualification
- Degree in Social Work (or equivalent)
Registration
- Registered with Social Care Wales
Experience
- Minimum 2 years’ post-qualified experience (including completion of ASYE or equivalent)
- Experience working with adults experiencing mental health difficulties
- Experience managing complex caseloads and high-risk situations
- Experience completing:
- Mental health assessments and care planning
- Risk assessments and safeguarding work
- Reviews and ongoing support planning
- Experience working within a community or multi-disciplinary mental health setting
- Experience working with carers and wider support networks
Legislation Knowledge
Strong working knowledge of:
- Mental Health legislation and policy
- Care Act 2014
- Mental Capacity Act 2005
- Safeguarding Adults procedures
- Relevant social care legislation and guidance
Skills
- Strong assessment and risk analysis skills within a mental health setting
- Ability to manage a demanding and varied workload effectively
