Integrated Lead – ICAT (Intermediate Care and Assessment Team) - Office base: Fylde and Wyre
About the Role…
As an Integrated Lead, you will:
- Provide operational leadership across ICAT services.
- Ensure effective coordination of care pathways, supporting timely interventions and safe transitions.
- Work collaboratively with multidisciplinary teams to deliver integrated, outcome-focused support.
- Drive service improvements aligned with Adult Social Care priorities and statutory requirements.
Key Responsibilities...
- Lead and manage staff within the integrated teams, fostering a culture of collaboration and continuous improvement.
- Monitor performance and quality standards, ensuring compliance with policies and best practice.
- Support strategic objectives for short-term interventions, reducing hospital admissions and promoting independence.
- Engage with stakeholders, including health partners, to strengthen integrated working.

You must be registered (SWE) to be appointed as a Social Worker or have applied to register with the Social Care Council.
