Job Description & Person Specification >Adult Social Care are currently looking to appoint Social Workers and a Social Care Assessors.
Do you have the right skills in delivering intensive support to individuals to enable them to provide the very best environments to Adults in the community to enable them to thrive?
We are seeking experienced Social Workers to join our dedicated team. In this vital role, you will be responsible for ensuring the safety and well-being of individuals within our community.
The ideal candidate will possess a strong background in social work practices and a deep understanding of Adult Social Care policies and regulations. Your role will involve conducting thorough assessments, collaborating with multidisciplinary teams, and developing comprehensive intervention plans to support at-risk individuals. You will be tasked with not only identifying and addressing immediate concerns but also empowering people to achieve long-term stability and well-being. You will demonstrate exceptional communication skills, case management expertise, and the ability to navigate complex emotional scenarios with empathy and professionalism.
Excellent recording and report writing skills and IT are required for this role.

Skills
Essential:
- Ability to exercise professional judgement within statutory frameworks and clearly evidence decision making rationale.
- Ability to apply range of Adult Social Care legislation, proportionality and least restrictive practice in complex situations.
- Ability to present as an excellent role model for good practice within the multi-disciplinary team and engage in reflective practice.
- Ability to provide information and advice around hospital discharge planning to include funding, and commissioned services in social care.
- Ability to take a lead role in jointly worked complex cases.
As this post meets the requirements of the Immigration Act 2016 (part 7), the ability to converse at ease with members of the public and provide advice in accurate spoken English is essential for the post.
Knowledge
Essential:
- Knowledge of Making Safeguarding Personal and strengths based social work practice.
- Knowledge of professional standards and regulatory requirements relevant to Social Work England.
- Advanced and detailed knowledge/skills of Care Act.
- Proven knowledge/skills of Mental Capacity Act.
- Knowledge of Managing Expectations.
Experience & Qualifications
Essential:
- Relevant professional Social Work qualification – Degree/Diploma Work, Certificate of Qualification in Social Work, Certificate in Social Services.
- Registered with professional body Social Work England.
- Commitment to continued professional development.
