Health and Social Care Coordinator
Salary: £28,000 - £30,000 | Location: South Tyneside | Type: Full-time, Permanent
Reed is recruiting on behalf of a values-led care organisation for a Health and Social Care Coordinator to support the day-to-day operation of services for adults and children with learning disabilities. This varied role combines care coordination, administration, compliance, and quality assurance, helping to ensure safe, effective, and person-centred support.
Key Responsibilities
- Coordinate daily service delivery, staffing schedules, and rotas.
- Maintain accurate care plans, risk assessments, and care records.
- Ensure compliance with CQC regulations, safeguarding requirements, GDPR, and health & safety standards.
- Support audits, quality assurance activities, and continuous improvement initiatives.
- Build positive relationships with service users, families, healthcare professionals, and external agencies.
- Investigate, record, and monitor incidents, complaints, and safeguarding concerns.
- Provide direct care and practical support when required.
What We're Looking For
- Previous experience in health and social care coordination, administration, or support services.
- Knowledge of care planning, safeguarding procedures, and CQC requirements.
- Strong IT skills, including Microsoft Office and electronic care management systems.
- Excellent organisational skills with the ability to manage competing priorities.
- Full UK Driving Licence and willingness to undergo an Enhanced DBS check.
